Platinum credit card account, issued by World Financial Network National Bank (WFNNB), WFNNB's finance charges on
the Total Amount Financed for the promotional credit plan purchase, are assessed on promotional credit plan purchases
from the date of purchase through the end of the promotional period. Provider-assessed finance charges
(typically at the time of purchase) are not those of WFNNB; rather, Provider-assessed finance charges are
part of the Total Amount Financed upon which WFNNB's finance charges are based. The minimum payment will be the
Total Amount Financed including the calculated WFNNB finance charges that will be assessed from the date of
purchase through the end of the promotional period divided by the number of months between the date of the purchase
and the promotional period end date (minimum $75.00) rounded up to the nearest whole dollar. If the Total Amount
Financed is not paid in full before the promotional period ends or regular revolving and/or any promotional plan
payments are not paid when required for two consecutive billing cycles, under this or any other MedChoice Platinum
promotional credit plan, the remaining balance and the WFNNB finance charges will be included in your regular
