online, or call us and receive an instant decision so that you may book a date with
your provider of choice.
What do I do with my Choices Card?
Once you receive your Choices card, you’re free to use it over and over for the healthcare
services you choose.

You may apply online, call us, or ask your provider for a medchoice application.
The application is fast and easy, and takes about 2 minutes to fill out.

Do I need to choose a doctor before I apply?
You can complete your application without having selected a provider. When you do choose a
provider, just bring in your approval sheet or choices card – your provider will call us and
handle the rest!
Do I qualify for MedChoice?
Approval is based on information from your credit application and past credit history.
When will I receive my first MedChoice billing statement?
You won’t receive a billing statement from MedChoice until a charge has been processed on your
account. Once a charge has been processed, you will receive your first billing statement
within 30 days.
Can I pay off my Choices account before the end of the term?
Yes. There are no pre-payment penalties.
I already have a Choices Card, can I use it again?
Yes, you can use your Choices Card over and over. As long as you have the available
credit on your account, you can use your card for ongoing or new treatments and procedures,
without reapplying.
